Porter-Leath's Family Service Workers share a special relationship with our children and their families. Our Family Service Workers provide effective social work and case management services to meet the individual needs of each child we serve. They ensure our families possess all of the resources they need to be healthy and thrive.
Our Family Service Workers:
• Use their knowledge of community resources and ability to prioritize tasks to manage caseloads effectively and independently
• Possess a strong conviction about the capacity of people to grow and change
• Forge mutually respectful partnerships with the children and families we serve
• Set limits and maintain the helping role of the employee and intervene appropriately to meet needs of children and families
Essential Job Functions
1. Performs duties according to the standards set forth by Porter-Leath, Head Start, the NAEYC, CWLA, and Tennessee Day Care Licensing.
2. Assists with completion of the application/enrollment process with participating families. Assists with the completion of social histories, nutrition assessments, and other health forms. Makes follow-up with community referrals to ensure eligibility to participate in the program. Ensures files are complete and updated.
3. Assists with providing extensive case management services required by families involved in Head Start to include: ensuring that enrollees have access to needed services. Ensures all clients enrolled have access to medical care. Develops and implements follow-up plans. Works with parents to promote parent participation in the delivery of services for their children.
4. Assists with recruitment of families with children ages three (3) to five (5). Provides information to the community about the program as assigned.
5. Assists with the completion of social histories.
6. Assists with the timely completion of professional, accurate, and thorough progress reports.
7. Assists participants in locating and enrolling in life skills training classes/workshops that will meet their particular needs.
8. Makes home visits to participating children/families as assigned.
9. Keeps Coordinator and Site Manager informed of programmatic issues in a timely and concise manner.
10. Performs research/study to remain current of the trends involving early childhood education, the needs of children at risk, and services to families affected by substance abuse.
11. Contributes to team efforts by performing other duties as assigned.
• Associate's degree in social services field or related field required.
• Bachelor's degree in social services field or related field preferred.
• Two (2) years of experience in a social service or educational setting.
• Head Start experience preferred.
